Name Change Process
Follow these numbered steps to file and complete an adult name change petition in Orleans Parish:
1. File Petition with Civil District Court
File your formal petition for name change with the Civil District Court Clerk’s Office, located at 421 Loyola Avenue, Room 402, New Orleans, LA 70112.
2. Serve the District Attorney’s Office
Formally serve a copy of the filed petition on the Orleans Parish District Attorney’s Office as required by Louisiana law.
3. OPDA Statutory Review
OPDA conducts the required review, including checking background information and verifying that no statutory prohibitions apply under La. R.S. 13:4751.
4. OPDA Response or Objection
If no legal bar exists, OPDA may file an answer or waiver with the court. If statutory prohibitions exist, OPDA may file a formal objection.
5. Contact Court for Next Steps
After OPDA files its response, the petitioner contacts Civil District Court to present the judgment or schedule a hearing.
